LIMIT: One per person, cannot have received a Fitbit previously from the Population Health, Innovation, and Informatics Unit at the Health Department.

REQUIREMENT: Must be a resident of Maryland.

PICK-UP:

• Pick-Up: Fitbit’s must be picked up at one of the two locations/times:
  • Pick-Up Option #1: Tuesday, February 18th – 3:00 p.m. to 5:30 p.m. – Garrett County Health Department in Oakland (Ask for Shelley)
  • Pick-Up Option #2: Friday, February 21st – 12:30 p.m. to 5:30 p.m. – Grantsville Branch of the Ruth Enlow Library (Held in Conjunction w/ the Food Voucher Program, Ask for John)

*Orders not picked up by 5:30 p.m. at either location will be forfeited. This will not exclude you from future programs.
**Participants must sign for Fitbits in person.
